COMMERCIAL INSURANCE (Through Headway)

Can be used for: Individual or Family Therapy
Cannot be used for: Reunification Therapy, Therapeutic Supervised Visitation
 

What to Expect:

• Headway manages billing and insurance verification

• Copays, deductibles, and coinsurance apply

• Diagnosis required

• Benefits determined by your specific plan
   

You Choose This If:
✔ You have coverage through Anthem/Blue Cross Blue Shield, Aetna, Cigna, Optum, Select Health Colorado, Quest Behavioral Health, Carelon Behavioral Health or United Healthcare
✔ You prefer in-network benefits
✔ You are comfortable using insurance
 

OUT-OF-NETWORK BENEFITS (OON) with Thrizer

Can be used for: Individual or Family Therapy
Cannot be used for: Reunification Therapy, Therapeutic Supervised Visitation
 

What to Expect:

• You pay the full session fee upfront until you meet your deductible. After deductible, options are available to pay only your co-pay up-front and Thrizer submits claims for reimbursement.

• Reimbursement depends on your insurance plan

• Thrizer charges a small service fee
   

You Choose This If:
✔ You have strong OON benefits or have an insurance not accepted through Headway.
✔ You want flexibility but still seek reimbursement
✔ You are comfortable paying upfront until your deductible is met
 

MEDICAID

Can be used for: Individual or Family Therapy
Cannot be used for: Reunification Therapy, Therapeutic Supervised Visitation
 

What to Expect:

• Services billed directly to Medicaid

• No out-of-pocket session cost (if eligible)

• Diagnosis and medical necessity required

• Session structure guided by Medicaid regulations
   

You Choose This If:
✔ You have active Medicaid coverage
✔ You prefer no out-of-pocket cost
✔ You are comfortable with insurance-required documentation
 

PRIVATE PAY

Can be used for: Individual or Family Therapy
Must be used for: Reunification Therapy, Therapeutic Supervised Visitation, PRE Work
 

What to Expect:

• Flat rate: $180 per session

• No insurance billing

• No diagnosis required unless clinically indicated

• Flexible treatment structure
   

You Choose This If:
✔ You prefer not to involve insurance or have a diagnosis
✔ Your goals are relational in nature or court ordered but not medically necessary
✔ You want the most flexible therapy structure

• Switching options requires advance notice and updated paperwork.
